Twilio Release and Authorization for Promotional and Advertising Material

I authorize Federated Mutual Insurance Company, its affiliates, successors and assigns, and association partners (collectively, “Recipients”), to reproduce, distribute, copyright, use, and publish any quotations, photographs, audio-visual footage or other personal expression, regardless of form, (collectively, “Materials”) which I send to Recipients. I acknowledge that such use is in the sole discretion of Recipients listed above.

I acknowledge I am providing my personal mobile phone number to Federated Mutual Insurance Company for the purpose of sending photos and/or videos via SMS/MMS (included within the term “Materials”) which I send to a company-managed number operated by Twilio. I understand that this number is managed by an automated system and that messages may be stored or processed for business purposes.

By submitting a photo or video using the company-managed Twilio number, I consent to Recipients’ use of that photo or video for advertising and public relations purposes, which includes consent on behalf of any member of my family, including minor children, that appear in the photo or video. I understand that standard message and data rates may apply.

I also release Recipients from any claims, complaints, damages, costs, or causes of action which may arise from Recipients’ reproduction or use of any Materials, subject to this release and authorization.
