DocuSign® Exclusion Process

This page details the DocuSign exclusion process with Federated Insurance.

HR Representative

This section is for the HR Representative of the company who requested the MVR. It explains what information is needed to be filled in order to start the DocuSign exclusion process.

NOTE: If the individual listed on the endorsement is no longer employed, please contact the Client Contact Center at clientcontactcenter@fedins.com or 1-888-333-4949.

  1. Within the received email, click “REVIEW DOCUMENTS”.

  2. Once the document has been opened, click the agree statement.

  3. The system will prompt you to Enter Recipient Information. Fill in the appropriate fields for the Authorized Representative and Excluded Person. 
    1. Name / Email – Required
      1. Enter the employee’s business e-mail. 
      2. If a business e-mail is not available, enter employee’s personal email.
    2. Private Message – Optional
    3. Click “FINISH”

  4. A notification will come up alerting the user that all recipients have been assigned. Click “DONE”.

    NOTE: Once you click “DONE”, the DocuSign envelope will be sent to the Authorized Representative to obtain their signature.

Authorized Representative

This section is for the Authorized Representative of the Named Insured. It explains how to sign the DocuSign exclusion form.

  1. Within the received email, click “REVIEW DOCUMENTS”.

  2. Once the document has been opened, click the agree statement.

  3. Click "CONTINUE"

  4. Follow the attached steps to complete your e-signature.
    1. Click “START”

    4. Adopt Your Signature.

      1. Enter the Required Fields (Full Name / Initials)

      2. Add signature (SELECT STYLE or DRAW)

      3. Click “ADOPT AND SIGN”

    5. Click the “Sign” button showing the red down arrow.
      Click Sign 2
    6. Click "FINISH"
      Click Finish
  5. A notice will come up to let the signer know signing has been completed. Click “CONTINUE”.

NOTE: Once this information is entered, the DocuSign envelope will be sent to the Excluded Driver.

The user can close DocuSign.

Excluded Person

This section applies to the person being excluded. It explains how to sign the DocuSign exclusion form.

  1. Within the received email, click “REVIEW DOCUMENTS”. The body of the email will include an Optional Private Message as well as a message/instruction from Federated.

  2. Once the document has been opened, click the agree statement.

  4. Adopt Your Signature.

    1. Enter the Required Fields (Full Name / Initials)

    2. Add signature (SELECT STYLE or DRAW)

    3. Click “ADOPT AND SIGN”

The user can close DocuSign.

FAQ

Q: What is the timeframe before we need to send an e-signed Driver Excluded form back to you?

A: We will follow-up 3 times (every 10 days) in the first month, then we will ask your marketing representative to contact you if we have not yet received the Driver Excluded form. 

 

Q: Will we need to re-sign all prior Driver Excluded forms?

A: Yes – we will send these to you after you have completed DocuSign testing and are ready to sign Driver Excluded forms.  E-sign the Driver Excluded forms for those drivers still employed.

 

Q: Whom do we contact if we have questions on DocuSign or driver insurability?

Q: What do we do if the driver is no longer employed or was not hired?

A: Contact Federated’s Client Contact Center (CCC) at SAService4@fedins.com or 1-888-333-4949

 

Q: Can we see the MVR?

A: No – due to privacy laws and contracts with our MVR vendor, we cannot share MVRs directly with you.  Please obtain a copy of the driver’s MVR, send the MVR to Alex, then she can discuss what is on the MVR you obtained.

 

Q: Do we get a copy of the signed exclusion?

A: Yes – in addition, you will get the Certificate of Completion.
